I am confused whether the term 'poster boy' refers to someone or a cause as a fine example thereof or whether it is a pejorative or manipulated to evoke false sympathy

Looking at a number of uses, I believe it is generally used to mean "a good example," though it could be a good example of something good or a good example of something bad. The false sympathy angle is not one that appears with any kind of frequency.
