A Perfect Storm

I hear the phrase "a perfect storm" or "a perfect storm of X" rather often, and I've been wondering where this phrase came from. I called, but decided to post here also. I can't believe it originated with the movie, but I've no clue where to look.

Thanks in advance for any thoughts!
