I would certainly have thought "it's a matter of fact..." would be up there.

"literally" when it's not
At work, a woman would always use the word "virtually" in meetings. Often, "literally" could've been substituted and that would've been equally annoying.
Her followup phrases included "irregardless". Luckily, she no longer works with us.
